Herbs treat SIBO as effectively as antibiotics

Small intestinal bacterial overgrowth (SIBO) is a pesky issue which means that there’s abnormal numbers of normal bacteria in the gastrointestinal tract. SIBO may cause intestinal symptoms ranging from diarrhoea and weight loss to anaemia and malabsorption. It’s believed to cause/aggravate symptoms of irritable bowel syndrome (IBS) [1] and some, like Chris Kresser, say it’s closely related to histamine intolerance, so dealing with SIBO may “alleviate” the former [2].

SIBO is diagnosed via lactulose breath testing (LBT), which is fancy speak for drinking a little sugar and then breathing into a tube.

As an aside, an interesting study published in Gut, the official journal of the British Society of Gastroenterology, raised the point that people administering this test may not properly trained in its execution nor its interpretation [3].

Many view SIBO eradication as the Holy Grail of histamine intolerance treatment. Kill the overgrowth, heal the gut, return to a normal diet. Sadly, spending months or years on antibiotics and or sugar free diets seems to be the norm.

I was lucky. I tested positive, started drinking olive leaf tea, cut most processed sugar out of my diet, but kept fruit in there. Six months later I was tested again and boom, it was gone.

Till now I haven’t really had a great reply to folks asking me how I managed to get rid of it. I never really thought much of it at the time because I didn’t really notice that much change in my symptoms afterwards. Or none that I attributed to the SIBO clearing up. Thing is though, and this is one of the biggest “mistakes” I made in my healing: I made too many changes at once, so it was hard to tell what had done what.

In this case, around the time I nixed the most commonly prescribed medical treatment; an antibiotic called Rifaximin and made the SIBO diet changes, I also started eating a diet very, very, very high in herbs. People’s eyes would bug out when they saw the volume of greenery going into my soups.

Turns out my instincts were right on, because herbs, some common culinary ones and others you might have to make a trip to your local Whole Foods supplement shelf for (under medical supervision of course!), have been found to treat SIBO as well as a 10-14 day course of the antibiotic Rifaximin, with significantly fewer side effects; like causing further bacterial imbalance in the body and the very scary super bug Clostridium difficile.

I encourage you to print out the study (thanks Adele for bringing it to my attention!) found here for your doctor, specifically TABLE 5 (you’ll have to click on that separately) which shows all the effective herbal remedies.

A few highlights from the remedies, which I’m totally biased in choosing because I eat or take these regularly, including them in my books. They’re the cornerstone of my healing process…

    I had a SIBO test last year but at the time I was already on a lowcarb Ketogenic diet. It came back negative unsurprisingly. I still think I may have SIBO as I do have sensitivity to high fodmap foods. Thanks for the list of herbs…I’ve already ordered berberine as I heard its good for depression too. Have you ever tried oregano oil? Some say it’s as strong as medical antibiotics. i find it a bit too strong.

    If someone reacts very badly to salicylates how do they treat their Sibo naturally?

    Hi Jennifer, as usual, sals stump me I’m sorry. I’m under the impression that the herbs I listed, when fresh, aren’t high salicylate though? I may be wrong, I haven’t looked at the list in ages.

    This post is perfect timing for me! Unfortunately I tried a few of the herbs, recommended (oregano/berberine) in a product called Candibactin AR/BR that didn’t work, Then I tried a stint with Rifaximin Which didn’t work either… I took a break, did further testing realizing my liver/gall bladder were struggling, and not producing enough glutathione. Do you think this could be the reason my SIBO would keep coming back? … I am very keen to try the following herbs now that I am addressing the liver/ gall bladder issue (plant tannins, Uva-ursi, berberine, and oregano)

    Hi! If green or black tea gives me a horrific migraine will olive leaf tea have the same side effect? Are we to make the preparation in the table? How so? Did I miss something in the article? : ) Curious if I should try glutathione. Gall bladder gone and from different natural doctors have said my liver isn’t detoxing well. I posted somewhere else as well if spirulina was o.k. for us. Don’t believe it was replied to. Forgive me if it was.

    Hi love, no it shouldn’t. Those teas are fermented, on purpose, and can be an issue for people with oxalate issues. Caffeine could also be a problem. I would print off the table for your doctor/nutritionist to help you with. Spirulina is a mast cell stabiliser but some will still react to it.

    No problem, really helpful none the less, checked that study you linked and bile acids was one of the protective factors (table 3) … so I think there is a link there. Detox and gut health seem to be connected.

    Hi Johanna, how did they know you weren’t detoxing properly? I have a good hunch that detox issues and SIBO are related. I am doing more research on this. Will be talking to my functional doctor, and Nutritionist as well. Did you do an Organic Acids Test?

    Originally, I had gone in for migraines and joint/muscle pain. Drs were certainly not listening to me. I was placed on a detox shake, adrenal support, and something for mitochondria. All with an elimination diet. I had to start with half a scoop for a week, stop for 3 days to rest or else I would have a huge crash, increase to a full scoop, repeat until I got up to 2 scoops. I found out so many foods that bothered me and felt a ton better. But, financially I was unable to continue. Plus, much of what they gave me I had to get off. Reacted to everything as I often do with scripts. They couldn’t help anymore. Plus, none could help me STOP reacting to so much nor could they explain why I was reacting to what they were giving me. Beyond excited to find so many simularities between Yasmina and myself. Hoping to move on to the next step. ? I can’t recall where in the process they mentioned my liver. Gall bladder is gone.

    I was the same way with the Oil, do you think your reaction was a herxheimer ? or was it longer than that?

    Ask about the Organic Acids Test it will show you a better picture of your dextoxification. Apparently the problem with a lot of shakes is that they do not give you enough of what you may need. Anyhow after reading the table 3 in the study I am certain that liver detox/ gall bladder function is crucial in this SIBO equation.

    I think I’ll print out that study and take it to the gastroenterologist when I see him next. He was very clear that antibiotics are the ‘only way’ to get rid of SIBO. I’ve had herbal treatment with the help of a nutritionist and was waiting for my follow up breath test result when I saw him. I’m pleased to say it came back negative!

    It would take some convincing to get me to try berberine in a recipe, though. I had a leaky capsule in my bottle and it really tastes awful! And it affected my bowels – not in a good way. I hope I never have to see that stuff again!

    SIBO for many people has a high relapse rate. Especially for people who have anatomical abnormalities due to surgery or hernia’s or other such.

    Simpler cases are easier to treat.

    Hi Yasmina. After the rifaximin treatment did you do stop eating beans and legumes for a while? All the sibo diets suggest to cut legumes as they are highly fermentable. It is also interesting that you started to increase your garlic and onion intake while these are again off the list for sibo. Can you please comment on this. Thanks. Peachy

    Oregano oil is so strong it will burn your skin when used pure. To take it in, it must be diluted with another oil, like olive or macadamia, and then placed in a capsule to be swallowed. Stomach acid will neutralise the burning effect of it. But must be diluted. I heard of some great results with it, though.

    I was tested for SIBO and was sure my results would come back positive because I had a very definite reaction to the lactulose they gave me to drink – I became, very, very, very sleepy. When they told me my results were negative, I was shocked and crushed. I thought I finally had an explanation and a chance at restoring my diet to somewhat normal. They had told me that I wouldn’t react to the lactulose before the test because the particles are too big to get through the intestinal wall and enter the bloodstream. So, I Googled it all after the fact and learned that doctors actually use lactulose to test for intestinal permeability. If they find it in your urine after you ingest it, it’s proof that you have IP. So, though they didn’t test my urine that day, I feel I have my proof that I have IP. Now I just need to figure out what is causing it and how I can heal it…..

    This article is good but SIBO is a symptom not a disease. SIBO is usually a symptom of slow motility of the intestinal tract. You can kill off the dysbiosis, but if you don’t restore gut function and motility, the SIBO will likely reoccur

    Yasmina, I have a feeling black seed oil is among those herb oils that would work for SIBO, though it has not yet been recognized for this purpose, and played a part in your recovery from SIBO.

    I found I can’t tolerate oregano oil, but I can take black seed oil.

    Thanks! I’m finding Swanson’s oregano oil softgels are too strong for me, will try diluting. Also, I hear that freezing the oil reduces the burping (which was a dealbreaker for me).

    Yasmina…there are two other herbs lots of us used a lot (along with you) that are potent antimicrobials and those are black seed (and the oil, especially) and holy basil. They weren’t used in the above study, but they undoubtedly played a part…certainly they have for me. So many different herbs have antibacterial properties.
